Liability Waiver

I hereby release Roxane Logereau from any liability or claims that may arise concerning my mental and/or physical well-being during the work outlined and agreed upon—both now and in the future—by completing this form. This liability waiver does not exclude or limit liability for death or personal injury caused by negligence.

Scope of Practice

I understand that Roxane Logereau is not a licensed physician, psychologist, or medical practitioner of any kind. I acknowledge that hypnotherapy is not a substitute for professional medical or psychological advice, diagnosis, or treatment from a qualified psychiatrist, psychologist, psychotherapist, or doctor.

Participation

I give Roxane Logereau full permission to hypnotize me and to use Rapid Transformational Therapy® (RTT®). I understand that my active participation in the process—including listening to my personalized recording for 21 days—is essential to achieving the best results.

Guarantee

I acknowledge that while Rapid Transformational Therapy® has a high success rate, Roxane Logereau cannot guarantee specific outcomes. My personal success depends on various factors beyond Roxane Logereau’s control, including my own commitment, mindset, and willingness to embrace change.

Audio Recordings

I grant Roxane Logereau full permission to create audio recordings that may include my voice. I understand that if a recording is made during or after my session(s), Roxane Logereau retains full copyright over any media produced and distributed to me.

Deepening Process

I consent to Roxane Logereau respectfully lifting my arm, touching my shoulder, or rocking my head during my Rapid Transformational Therapy® session(s) to facilitate the deepening process.

Confidentiality

By signing this form, I acknowledge that Roxane Logereau may release information to a designated individual or agency under the following circumstances:
•    If it has been determined that a vulnerable person (child or adult) is at risk.
•    If I am deemed to be in imminent danger to myself or others.
•    If a subpoena for records has been issued.

 

Additionally, I understand that Roxane Logereau may discuss aspects of my case with professional colleagues while maintaining my full anonymity unless I provide explicit permission otherwise.

Cancellation policy 

To ensure the best experience for all clients, I kindly ask for advance notice if you need to cancel or reschedule your session:
•    Please provide at least 48 hours’ notice for cancellations or rescheduling of an RTT session.
•    No-shows or cancellations with less than 48 hours’ notice will incur a $50 late cancellation fee.

 

For complimentary discovery calls, I understand that life happens! If you need to reschedule, please provide as much notice as possible so I can offer the time to someone else. While there is no charge for missed calls, your courtesy is greatly appreciated.

 

I understand that unexpected situations may arise, so please reach out as soon as possible if you need to make changes.
